Lipoedema Surgery: Liposuction for Lipedema Treatment

Lipoedema (lipedema) is a chronic, painful disorder characterised by the symmetric accumulation of pathological fat tissue in the legs (and sometimes arms) — almost exclusively in women. It is distinct from obesity: diet and exercise do not reduce lipoedema fat. Surgical liposuction can significantly reduce volume, pain and progression.

What Is Lipoedema?

Lipoedema fat is histologically and functionally different from normal fat. It causes:

  • Disproportionate, symmetric enlargement of legs (and sometimes arms)
  • Pressure pain and bruising tendency
  • Resistance to diet and exercise
  • Progressive worsening without treatment

Stages

  • Stage 1: smooth skin surface, increased subcutaneous tissue
  • Stage 2: uneven skin (mattress-like), nodular fat tissue
  • Stage 3: large lobular fat deposits, skin folds

Surgical Treatment

Water-Jet Assisted Liposuction (WAL / Body-Jet)

Preferred method for lipoedema. A fine fan-shaped water jet dislodges fat cells while simultaneously suctioning them out, with less trauma to surrounding tissue. Preserves nerves and lymph vessels — important as lipoedema patients have compromised lymphatics.

Tumescent Liposuction

Also effective. Local anaesthetic solution infused to numb and firm the tissue before suctioning.

Costs

ProcedureCost per area/session
WAL liposuction (both legs, one region)€3,000–€6,000
Full leg treatment (multiple sessions)€8,000–€20,000

Note: In Germany, lipoedema liposuction has been covered by statutory health insurance since 2020 for Stage I–III under defined criteria.
