Brazilian Butt Lift: Natural Buttock Enhancement with Own Fat

The Brazilian Butt Lift (BBL) is a surgical procedure that uses the patient's own fat (autologous fat transfer) to augment and reshape the buttocks. Fat is harvested by liposuction from areas such as the abdomen, flanks or thighs, processed and purified, then injected into the buttocks to create volume, projection and improved shape.

Procedure

  1. Liposuction: fat is harvested from donor areas (typically abdomen, waist, thighs)
  2. Purification: harvested fat is processed by centrifugation or filtration to separate viable fat cells
  3. Injection: purified fat is injected in multiple small aliquots throughout the buttock tissue

Safety: Critical Considerations

BBL has historically had the highest mortality rate of any cosmetic procedure — estimated at 1 in 3,000 — due to fat embolism risk when fat enters the gluteal venous system. Modern safe BBL protocols require:

  • Injection only into the subcutaneous (fat) layer — never into the muscle
  • Use of blunt cannulas to reduce vascular injury risk
  • Performed by a board-certified plastic surgeon
  • Real-time ultrasound guidance in some protocols

Since protocol changes, mortality risk has decreased significantly. Choosing an experienced, properly trained surgeon is paramount.

Results

Approximately 50–70% of transferred fat survives long-term. Initial result appears larger due to swelling; final shape visible at 3–6 months. Long-lasting if weight is maintained.
