Rosacea Treatment: Medical and Cosmetic Options for Facial Redness

Rosacea is a chronic inflammatory skin condition causing facial redness, visible blood vessels, and in some cases papules, pustules and skin thickening (phyma). It primarily affects the central face — nose, cheeks, chin and forehead — and worsens in flares triggered by various factors.

Types of Rosacea

  • Type 1 (ETR — Erythematotelangiectatic): persistent redness, flushing, visible capillaries
  • Type 2 (Papulopustular): acne-like papules and pustules with redness
  • Type 3 (Phymatous): skin thickening, especially on the nose (rhinophyma)
  • Type 4 (Ocular): eye irritation, dryness, redness

Treatment Options

Topical Treatments

  • Metronidazole gel/cream: reduces papules and pustules
  • Azelaic acid (15–20%): anti-inflammatory, reduces redness and lesions
  • Ivermectin 1% cream: effective for papulopustular rosacea
  • Brimonidine gel: vasoconstrictor for temporary redness reduction (ETR)

Laser and Light Treatments

  • IPL (Intense Pulsed Light): reduces diffuse redness and visible vessels. Most effective for ETR. 3–5 sessions.
  • Nd:YAG laser / Pulsed dye laser: targets individual telangiectasias (visible vessels)
  • CO2 laser / surgery: for rhinophyma (nose thickening)

Trigger Management

Key triggers: sun exposure, hot drinks, alcohol, spicy food, temperature extremes, exercise. Identifying and avoiding personal triggers is crucial for long-term management.
