Mid-Face Lift: Surgical Rejuvenation of the Cheek and Midface

The mid-face lift (midface lift) specifically addresses the middle third of the face — the cheeks, the area under the eyes, and the nasolabial fold region. As we age, the fat pads in the cheek area descend, causing the characteristic "tired" or "sunken" mid-face appearance. A mid-face lift repositions these descended tissues upward.

Indications

  • Descended cheek fat pads — "heavy" lower face appearance
  • Deep nasolabial folds caused by tissue descent
  • Hollowing under the eyes due to fat descent
  • Good skin quality — best results when skin elasticity is present
  • Patients who have already had a lower face/neck lift

Techniques

Endoscopic Mid-Face Lift

Minimal incisions — 3–4 small cuts hidden in the hairline and inside the mouth. Camera-guided repositioning of the SMAS and cheek fat. Less downtime than open techniques. Best for younger patients with mild to moderate sagging.

Open (Composite / SMAS) Mid-Face Lift

Performed through the standard facelift incision (in front of the ear). Allows repositioning of both the SMAS layer and the malar fat pad together. More powerful correction for significant sagging.

Recovery

Swelling: 3–4 weeks significant, 3 months full resolution. Return to work: 2–3 weeks. Final results visible at 3–6 months.
