Ear pinning (otoplasty) is a surgical procedure to correct prominent or protruding ears. It is one of the most common procedures in aesthetic surgery and can be performed from approximately age 5–6 years (when the ear cartilage is fully developed) as well as in adulthood. The goal is a natural, harmonious ear position that no longer attracts undue attention.

Surgical Procedure

Ear pinning is performed under local anaesthesia (adults) or general anaesthesia (children). The operation takes approximately 1–2 hours per ear.

  1. Incision on the back of the ear (in the natural fold)
  2. Exposure of the ear cartilage
  3. Reshaping the cartilage (scoring, suturing or partial excision)
  4. Suturing: permanent sutures fix the new ear position
  5. Bandaging with protective head dressing

Recovery and Aftercare

  • Head bandage for 7–10 days
  • Headband at night for 4–6 weeks (to protect the result)
  • No contact sports for 4–6 weeks
  • First result visible immediately, final result after 3–6 months

From what age can ear pinning be performed?

Surgeons recommend waiting until approximately age 5–6 years when the ear cartilage has reached its final size and is stable enough for surgery. Earlier surgery is technically possible but not recommended. In adulthood, ear pinning can be performed at any age.

Are the results permanent?

Yes, with correct surgical technique the result is permanent. Very rarely, renewed protrusion can occur due to cartilage memory, which may require a minor revision.

Is ear pinning covered by health insurance?

For children with severe protrusion (cup angle >35°) and documented psychological stress, statutory health insurance may cover costs. For adults, this is generally a cosmetic procedure paid out of pocket.
