Overview: Thermage FLX

Thermage FLX is the most advanced iteration of the Thermage platform — a non-invasive monopolar radiofrequency skin tightening system from Solta Medical (part of Bausch Health Companies). FDA-cleared and CE-certified for non-invasive skin tightening and contouring of the face, neck, eyelids, and body. Thermage is one of the most extensively studied non-invasive RF technologies, with over 15 years of peer-reviewed clinical data and more than one million treatments performed globally.

The FLX (Flexible) generation represents significant technological advances over previous Thermage generations: the AccuREP technology provides real-time tissue impedance feedback for consistent energy delivery, the Total Tip 4.0 applicator offers a larger treatment surface for faster treatment times, and improved comfort vibration reduces treatment discomfort.

Monopolar Radiofrequency Technology and AccuREP

Thermage uses monopolar radiofrequency, which fundamentally differs from bipolar or multipolar RF systems. Bipolar RF creates a current between two closely spaced electrodes, limiting penetration to 2 to 3 mm (dermis only). Monopolar RF creates a current between the treatment tip and a return pad placed at distance on the body, allowing energy penetration to depths of 4 to 6 mm — reaching the deep dermis, subcutaneous fat, and superficial fibrous septae.

The target tissue (collagen-rich dermis and subcutaneous fibrous tissue) is heated to 65 to 75°C. This temperature causes two effects: immediate collagen contraction (visible tightening during treatment) and initiation of a wound-healing cascade that stimulates neocollagenesis over the following 3 to 6 months. The AccuREP technology measures tissue impedance at each treatment point and automatically adjusts energy output to ensure consistent, controlled energy delivery regardless of tissue variation — reducing both under-treatment and risk of thermal injury.

Indications

Facial skin tightening (cheeks, jowls, perioral, neck, submentum), eyelid tightening (a specific FDA-cleared indication not available for most other RF devices), body contouring and skin tightening (abdomen, flanks, inner arms, inner thighs, buttocks), improvement of cellulite appearance, and décolleté tightening.

Treatment Protocol

Thermage FLX typically requires a single session per year — distinguishing it from most other energy devices that require series of 3 to 6 sessions. Treatment duration: 45 to 90 minutes depending on the area treated. Anesthesia options: topical anesthetic cream, oral analgesics, vibration anesthesia (built-in comfort pulse), or nerve blocks for sensitive patients. No downtime. Patients return to normal activities immediately. Results develop progressively over 3 to 6 months as neocollagenesis proceeds. Duration of results: 1 to 2 years; annual maintenance treatment recommended.

Safety Profile

FDA-cleared and CE-certified with extensive clinical safety data. Common: temporary redness and mild swelling resolving within hours. Rare: prolonged erythema, blistering if energy is excessive (prevented by AccuREP). Contraindications: metallic implants in the treatment area, pacemakers or implantable defibrillators, pregnancy.
