Double Chin Removal: Causes, Treatments and Costs
A double chin — medically known as submental fat accumulation — is one of the most common aesthetic concerns that brings patients to a plastic surgery practice. The excess tissue beneath the chin can significantly alter the facial profile and impact personal well-being. Importantly, a double chin is not exclusively a problem for overweight individuals — even slim, athletic people can develop a pronounced submental fat pad due to genetics.
Modern medicine offers an impressive range of treatment options: from minimally invasive procedures such as cryolipolysis, ultrasound or radiofrequency treatments, to surgical interventions such as chin liposuction or neck lift. The choice of the optimal method depends on numerous individual factors — age, skin quality, severity of the double chin, and personal expectations, as well as the preference for a surgical or non-surgical approach.
What Is a Double Chin? Definition and Anatomy
A double chin refers to visible fat accumulation or skin laxity in the area beneath the lower jaw, creating the optical impression of a second "chin line." Anatomically, this zone lies between the chin bone (mandible), the hyoid bone and the neck structures.
The submental region is composed of several tissue layers:
- Skin: The outer layer, which loses elasticity with age
- Subcutaneous fat: The fat tissue directly beneath the skin (pre-platysmal)
- Platysma: The flat neck muscle that plays an important role in defining the chin-neck angle
- Deep fat: Fat tissue below the platysma (sub-platysmal)
- Submental glands and structures: Lymph nodes and submandibular salivary glands
Causes of a Double Chin
Genetic Factors
Genetic predisposition plays an outstanding role in the development of a double chin. Many people carry a submental fat pad despite a normal body weight and regular exercise — simply because their fat cell distribution is genetically programmed this way. This predisposition can barely be influenced by diet and exercise, making medical or aesthetic treatments the only truly effective option for many.
The bone structure of the lower jaw also has an influence: a weakly developed chin (retrognathia) or a flat jaw angle can make a double chin appear more prominent even when only a small amount of fat is present.
Weight Gain and Fat Distribution
In cases of excess weight, the body stores energy as fat — and the submental area is one of the typical sites for this accumulation. The fat in the chin-neck area often responds stubbornly to dieting: while fat is reduced elsewhere, the double chin tends to persist.
Skin Aging and Loss of Elasticity
With age, the skin loses its elasticity due to the progressive breakdown of collagen and elastin. Even in the absence of significant fat accumulation, the skin can hang downward and create the visual impression of a double chin. Rapid or significant weight loss can also lead to a double chin, as the skin cannot retract quickly enough to follow the fat reduction.
Platysma Changes and Posture
The platysma muscle spans the neck and provides a clear chin-neck contour when toned. With aging, the muscle fibers relax and can separate from the midline — a platysmal diastasis — creating vertical bands on the neck and contributing significantly to the double chin appearance. Modern habits such as prolonged smartphone use ("tech neck") may also contribute to early laxity in this area.
Non-Surgical Treatments for a Double Chin
Kybella / Belkyra (Injection Lipolysis with Deoxycholic Acid)
Kybella (marketed in Europe as Belkyra) is an injection-based procedure for dissolving subcutaneous fat cells in the chin area. The active ingredient deoxycholic acid is a naturally occurring bile acid that normally aids in the digestion of dietary fats. When injected directly into the submental fat tissue, it destroys fat cell membranes permanently. The destroyed fat cells are then transported out of the body via the lymphatic system.
Procedure: After thorough analysis and marking of the treatment area, multiple small injections are placed — typically 20 to 50 per session. Local anesthesia or cooling significantly reduces discomfort. The treatment itself takes 15 to 30 minutes.
Results: Generally 2 to 4 sessions spaced 4 to 6 weeks apart are required for optimal results. Results are permanent as fat cells do not grow back, though remaining fat cells can enlarge with subsequent weight gain.
Costs: Approximately 400 to 800 euros per session; total treatment (2–4 sessions) typically costs 800 to 2,500 euros.
CoolMini — Cryolipolysis for the Chin
CoolMini (a specialized applicator of the CoolSculpting system) is a non-invasive procedure that destroys fat cells through targeted cooling. At temperatures around 4 degrees Celsius, fat cells die while surrounding skin, muscles and nerves remain unharmed.
Procedure: A vacuum applicator is placed on the chin area and draws the fat tissue in, cooling it for approximately 35 to 45 minutes. The treated area is briefly massaged afterward.
Results: Develop over 2 to 3 months as the body gradually eliminates the destroyed fat cells. Clinical studies show a fat tissue reduction of 20 to 25% per session. Usually one or two sessions are sufficient.
Costs: 500 to 900 euros per session.
Endolift — Laser Fiber Lipolysis for Chin and Neck
The Endolift is a minimally invasive laser therapy introduced via very fine optical fibers (1.1 mm diameter) under the skin. The laser energy simultaneously dissolves fat tissue and stimulates collagen production — addressing both root causes of a double chin: excess fat and skin laxity. Recovery time is just 1 to 3 days. Costs: 1,200 to 2,500 euros.
HIFU — High-Intensity Focused Ultrasound
HIFU (e.g., Ultherapy, Ultraformer) is a non-invasive method for skin tightening. Focused ultrasound generates temperatures of 65 to 70 degrees Celsius in deep tissue layers, triggering immediate collagen contraction and stimulating new collagen formation over subsequent months. Treatment can reach depths of 1.5 mm, 3 mm and 4.5 mm (SMAS layer), comparable in effect to surgical tightening. Full results appear after 2 to 3 months and last 12 to 24 months. Costs: 800 to 1,800 euros per session.
Radiofrequency (RF) Skin Tightening
RF treatments use electromagnetic energy to heat deep skin layers, triggering collagen fiber contraction and new collagen production. Devices such as Thermage, Morpheus8 or Exilis Ultra are commonly used. Fractional RF with microneedling (Morpheus8) is considered particularly effective. Costs: 300 to 1,200 euros per session; typically 3 to 5 sessions recommended.
Thread Lift for the Chin
Thread lifting uses bioresorbable threads (PDO or PLLA) to mechanically lift and tighten excess skin while stimulating collagen production. Suitable for mild to moderate laxity; less dramatic than surgical neck lifting. Results last 12 to 24 months. Costs: 800 to 2,500 euros.
Surgical Treatments for a Double Chin
Chin Liposuction (Submental Liposuction)
Submental liposuction is the gold standard for surgical treatment of a double chin caused by excess fat. It offers precise, permanent and predictable fat removal. Performed under local anesthesia with or without sedation, fine cannulas are introduced through small incisions (2–4 mm) to aspirate the fat layer by layer. Modern techniques often combine classical liposuction with laser lipolysis (SmartLipo) or ultrasound liposuction (VASER) for simultaneous skin tightening. Recovery: return to work in 2 to 3 days; a compression garment is worn for 1 to 2 weeks. Final results visible after 3 to 6 months. Costs: 2,000 to 4,500 euros.
Neck Lift (Cervicoplasty)
A neck lift is the most comprehensive surgical procedure for treating a double chin and is recommended when significant skin laxity and muscle diastasis are present alongside fat accumulation. Incisions are made behind the ears and/or beneath the chin; the platysma is sutured in the midline and tensioned laterally; excess skin is removed. Combination with a facelift delivers harmonious, rejuvenated overall results. Recovery: 1 to 2 weeks for light activities; 3 to 4 weeks before sports. Costs: 4,000 to 9,000 euros.
Choosing the Right Method
- Mild double chin (little fat, good skin quality): Non-invasive options such as CoolMini, HIFU or Belkyra deliver good results with multiple sessions.
- Moderate double chin (moderate fat, mild laxity): Endolift, liposuction or Belkyra combined with HIFU or thread lift are appropriate options.
- Pronounced double chin (significant fat and/or severe laxity): Surgical methods — liposuction with platysmal plication or neck lift — deliver the best outcomes.
Young patients with good skin elasticity benefit most from pure fat-removal procedures, as their skin retracts well. Older patients with reduced skin elasticity often require a combined fat-and-tightening approach.
